NEW Player: $75 per player- 4 -9 year olds
$80 per player- 10 -18 year olds
RETURNING *from Fall 2011* Player: $60 per player- 4 -9 year olds
$65 per player- 10 -18 year olds
Players must be at least 4 years old as of December 31, 2011.
**New players to CCSA must bring proof of date of birth at time of registration.
If you're not able to come in person to register for the upcoming season, you can print off a registration form and mail to the address located at the top. Be sure to include a check or money order payable to CCSA. If your child has never played for CCSA please send a copy of their birth certificate.
Soccer games will begin Saturday, March 17
Remember, NO CO-ED teams and ALL teams are AGE PURE!

Secretary/Treasurer | Picture day is Saturday, October 15. Ultimate Exposures will be taking individual and team photos. CCSA ask that every player be photographed because we share team photos with sponsors and the rebate we receive helps pay for trophies. Here is some information regarding picture day:
• Two different ways to order photos:
#1 - Prepay with our company provided envelope system on picture day,
#2 - Try the no hassle, see before you buy online system! We photograph your athletes and
parents can then order at our website: www.ultimateexposures.com
• Professional quality photographs are delivered within 3 weeks (usually faster) of order placement.
• Buddy shots will be available for athletes to take with their families and friends. A separate order form (available at picture day) can be used for buddy shots OR you can wait to view and order them online.
• Every athlete must bring the order form to picture day regardless of whether or not they are ordering additional photographs. Athlete and shipping information must be filled out.
• Customer Service is available Monday through Friday online, through email or by phone.
• 100% satisfaction guarantee or your money back on all orders.
• Shipping charges apply on all orders. It will be a flat fee of $4.00.
