Secretary/Treasurer | CCSA is offering PRE-REGISTRATION for FALL 2009. Players who register on or before June 15th will receive a $10 discount!Players from the spring season should of received a pre-printed registration form, if so, review the information and make any changes to your address, phone numbers, or email if necessary. However, if you did not or are needing to register a new player, you can download a registration form .
Each player receives a new uniform in the fall, so it's very important that you include a uniform size. If your child has a special need, i.e. to be on same team with another player for transportation purposes, make that notation under Additional Information on the form. Players will remain with the same team they played for this spring unless 1) they are not old enough to move up with the core, 2) the team disbands and/or 3) a request not to return to same team is written on registration form. Registration should be mailed to the address listed at the top of the form and be postmarked by June 15th to be applicable for discount. *Note: Players whose registration forms are not accompanied with full tuition will not be assigned to a team.
Fees till June 15th:
1 player - $60
2 players - $120
Each additional player from the same family - $30
All players must be 4 years old as of July 31, 2009 and a copy of their birth certificate must be included with registration if they have not played with CCSA before.
